Please make sure to bring all of these forms with you to your first office visit.
Do not mail them back to the office. Also, please remember to always
bring your picture ID, current insurance cards and your co-payment. If your
health insurance requires you to select a primary care doctor please do so prior to
your office visit. Please bring in any and all medication you take, in their
original bottles, to your appointment.
If the patient is under 18 years of age he or she must be accompanied by an
adult and will need to bring a copy of their current immunization certificate.
Please arrive 15 minutes ahead of your scheduled appointment time so that if
you have questions about these forms or we need more information, we can
address it all prior to your appointment.

Dear Prospective Ed.D., Higher Education Strand Applicant:
We are very pleased that you are interested in the Higher Education Strand of CCSU's Doctor of
Education (Ed.D.) in Educational Leadership, designed for current higher education professionals
who aspire to leadership positions on college or university campuses. We look forward to receiving
your application.
As you complete your application, keep in mind the following admission criteria:
1. Master's degree from an accredited institution of higher education in a discipline or
professional field that is relevant to the Ed.D. in Educational Leadership.
2. A 3.00 or higher cumulative average (GPA) in all graduate coursework.
3. Two or more letters of reference from leaders in postsecondary education familiar with
your work. Ask your references to use the form on the next page.
4. Résumé that illustrates important work-related experiences with an emphasis on yo ur
work as a leader at postsecondary institutions of higher education.
5. Acceptable scores on the General Test of the Graduate Record Examination (GRE) taken
within five years of your application.
6. A personal statement covering six important topics:
• Career goals
• Intended area of individual specialization
• Reasons for pursuing a doctorate
• Commitment to residency requirements (one three-day weekend in the first spring
semester, one full week each of the first, second, and third summer sessions)
• Commitment to enrolling in two cohort courses each spring and fall semester
• Commitment to summer enrollment during each 8-week summer session
7. If selected as a finalist, a satisfactory interview with the admissions committee.
We accept new students in alternate years only. Applications are due by October 1, 2017.
Admission standards are rigorous, and not everyone who meets our standards wil l be accepted.
Please note that the admission process calls for submission of materials to two locations. The last
page of this packet is a checklist of the various steps. Submit your Graduate Application and $50
application fee online. Transcripts from every college you have attended as an undergraduate and
graduate student should be submitted to Graduate Admissions in 102 Barnard Hall. In addition you
must send the following materials directly to the Ed.D. Program (attention Rouzan Kheranian) in 320
Barnard Hall:

Dear MSU Faculty,
Over the past two years, Medical Services at University Health Partners (UHP) has been
discussing ending the practice of providing medical excuses (aka "doctors notes"). As a
reminder, MSU has no policy requiring faculty to make accommodations for absences,
except when students are representing MSU in athletic events or in other official capacities
(sec. 310.1 of the Student Conduct Code).
UHP discussed rationale for ending the practice of medical excuses with Academic Council,
Faculty Senate, and ASMSU Senate, among other groups. Reasons for updating UHP
practices, include:
Part of the university learning experience for students includes communicating
directly with a faculty member, in a manner similar to how they would need to
co mmunicate with their future employer, when they needed to miss a work
assignment.
Illness is only one possible "legitimate" reason for an absence (others might include
a family emergency, sick child, mandatory court appearances, jury duty, car trouble,
etc.).
Students who visit UHP for only a medical excuse create problematic consequences
for UHP and for other students because:
Visits by students merely seeking a doctor's note, use scarce resources that
that are needed for students who truly need medical attention.
Visits by students merely seeking a doctor's note can lead to unnecessary lab
tests and x-rays.
For public health reasons, UHP will recommend that students with a highly
contagious illness (e.g., influenza) not go to class and only seek medical care if
they have unusual symptoms or a co-existing chronic health condition.
Students who use the existing excuse system to &ldq uo;game" the faculty and avoid
their responsibilities are equally adept at misleading medical providers for
many illnesses that otherwise have no physical or laboratory finding
(migraines, abdominal pain, nausea, etc.). Medical providers need to trust
that a patient is providing an honest account of their illness in order to
accurately diagnose and treat them. If there is a secondary gain of being
excused from an assignment, class, or test by visiting the medical provider, it
calls that open, honest dialogue into question, and provides disingenuous
information to the faculty member.

Dear UHD Colleagues,
You already have received previous correspondence from me on the issue of Campus Carry.
This is a law that will no doubt drive ongoing conversations at the University and across the
state. As the first day of classes approaches, I know there may still be questions about
UHD's policies and procedures concerning the law. I certainly welcome and invite discussion
on the topic, and I am sending this message to offer some details that will assist you this
semester and going forward.
Much of this information has been previously communicated, but my intent is to offer this
message as an informational resource for both part-time and full-time faculty.
In my last message to you, I referenced language that can be used on syllabi. Please note
that syllabus notification is not required by the University or the state. Faculty can elect to
use the wording below that has been reviewed b y the UH System Office of the General
Counsel.
 As of August 1, 2016, the "Campus Carry Law" (SB 11) is in effect on the UHD
campus. Any individuals who hold a current and valid License to Carry may carry a
concealed handgun on or about their person, where permitted on UHD
property. Certain areas of campus have been designated as gun-free areas, and
there are notices or signs at those locations. Please refer to the UHD Campus Carry
Policy (https://www.uhd.edu/administration/employment-servicesoperations/resources/Documents/PS01A16.pdf) for a list of the areas designated as
gun-free and to understand your rights and responsibilities under the policy.
Again, the use of syllabus language is at your discretion. Please know that faculty members
cannot make statements on syllabi or course communications that would identify, limit,
restrict or dissuade those with licenses to carry from exercising their rights tha t are
provided in the University's Campus Carry Policy or applicable law. Faculty can review the
Texas code regarding wrongful exclusion of license holders here.

UHD faculty have been exemplary in leading the discussions, fashioning policy, and
